Request a Referral

For a new referral, please make an appointment with your PCP and discuss your request with him or her.

For an update on your referral, please call our referral coordinator at (623) 334-8670 extension #102.

**Please allow 8-14 days for your referral to be completed.

**If you know of a specialist that you would like to go to, please make sure that you call your insurance company and make sure that the physician is contracted with your insurance company, then advise the referral coordinator of this.
