The Patient's Checklist

1) Bring a valid form of of government issued ID

2) Current Health Insurance card (if you have insurance)

3) A list of all medications you're currently taking

4) If you're not sure that we accept you're insurance plan, please call ahead to confirm.

5) If you are a Tricare member, you must call Tricare to notify them of your visit to the our Urgent Care facility.
